Hereford Medical Group Publishes 2024/25 Annual Report

Today we are publishing our Annual Report for 2024/25, showcasing a year of achievements, innovation, and ongoing commitment to delivering high-quality, compassionate care to more than 47,000 patients across our community.

A Year in Numbers

From April 2024 to March 2025, HMG delivered:

  • 139,000+ face-to-face appointments
  • 118,000+ online medical requests
  • 27,000+ telephone appointments
  • 8,981 flu vaccinations plus thousands more Covid, shingles and childhood immunisations
  • 898,000+ prescriptions processed

Our appointment attendance rate remained high at 91.7%, and patient requests were responded to quickly, with urgent same-day cases managed promptly by our dedicated Urgent Care Team.

Key Highlights of 2024/25

  • Total Triage launched in July 2024, improving safety, access and efficiency by ensuring every request is reviewed by a GP
  • Pharmacy First saved more than 2,000 GP appointments by enabling patients to be treated directly by community pharmacists for certain conditions
  • Phone system upgrades with call-back options, multi-language support and shorter waiting times
  • Surgery site improvements including solar panels, flooring upgrades and better parking facilities
  • Expansion of clinical teams, including new GPs, nurses, paramedics and a wound management team
  • Ongoing projects tackling health inequalities, from improving cancer screening uptake to supporting LGBTQ+ patients

Looking Ahead – 2025/26

Over the next year, HMG will:

  • Increase GP sessions by around 10% by autumn 2025
  • Focus on continuity of care to strengthen relationships between patients and clinicians
  • Roll out ABtrace software to streamline long-term condition reviews
  • Continue improving telephone response times
